Resolution approving Amendment No. 4 to the agreement between City, acting by and through, the Department of Public Health (DPH), and San Francisco Public Health Foundation for community health engagement, to extend the term by one year and six months from January 1, 2025, for a total term of January 1, 2020, through June 30, 2026, and to increase the amount by $1,302,153 for a total not to exceed amount of $21,329,720; and to authorize DPH to enter into amendments or modifications to the agreement that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the agreement or this Resolution.
