Resolution approving Amendment No. 5 to the agreement between the City and County of San Francisco, acting by and through the Department of Public Health (DPH), and Urban Alchemy, to provide Community Response Team services, a community-driven initiative that provides rapid, compassionate, and structured responses to targeted, non-medical, non-emergency 911 and 311 calls involving people who are experiencing homelessness, to increase the agreement amount by $4,730,871 for a new total not to exceed amount of $14,630,871 with no changes to the contract term of May 1, 2023, through April 30, 2028; and to authorize DPH to enter into amendments or modifications to the agreement that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the agreement or this Resolution.
