Resolution approving the First Amendment between the City, acting by and through the Department of Disability and Aging Services, including on behalf of the Human Services Agency, and Homebridge, Inc. for the provision of Contract Mode In-Home Supportive Services, to increase the amount by $2,419,719 for a new total not to exceed amount of $144,684,989 effective upon approval of this Resolution, with no changes to the term of five years from July 1, 2020, through June 30, 2025; and to authorize the Department of Disability and Aging Services to enter into amendments or modifications to the First Amendment that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the First Amendment or this Resolution.
