Resolution approving the Program Funding Agreement (“PFA”) between the City and County of San Francisco (“City”), acting by and through its Department of Public Health (“DPH”), and UCSF Health Community Hospitals (“UCSF”), as Grantees, and the California Department of Health Care Services (“DHCS”) and DHCS’s third-party administrator Advocates for Human Potential, Inc., as Grantors; including Permitted Use for UCSF Health Hyde Hospital at 900 Hyde Street for a term effective upon the execution of the PFA, through June 30, 2030, with an amount not to exceed $70,893,613; authorizing DHCS to apply for a receiver in the event of the Grantees’ default under the PFA or the Regulatory Agreement and Declaration of Restrictions; and authorizing DPH to enter into amendments or modifications to the PFA that do not materially increase the obligations or liabilities of the City and are necessary to effectuate the purpose of the Grant.
