Resolution retroactively approving Amendment No. 2, authorizing the Executive Director of the Human Services Agency to execute an amendment to the Grant Agreement between the City and County of San Francisco, by and through its Human Services Agency, and the non-profit Self-Help for the Elderly to provide the Nutrition Services Program to older adults and adults with disabilities for the total agreement term of July 1, 2017, to June 30, 2020, by increasing the agreement amount by $1,136,712 with a contingent amount of $960,075 for a total not to exceed amount of $10,560,828.
