Resolution authorizing the San Francisco Department of Public Health (DPH) to accept and expend retroactively a two-year grant from Kaiser Permanente in the amount of $150,000 for the project entitled “Shape Up BVHP,” for the period of October 1, 2007 through September 30, 2009, which includes a subcontract with the SF Study Center for $130,610 for the same time period.
